INVITATION FOR THE PRE-QUALIFICATION/EXPRESSION OF INTEREST UNDER THE CAPITAL PROJECTS FOR 2024 BUDGET
(1) INTRODUCTION
(1.1) The University College Hospital, Ibadan, Nigeria’s foremost Teaching Hospital, a parastatal under the Federal Ministry of Health, is to provide achievable, realistic and sustainable projects that will be beneficial to berth patients and members of staff in achieving excellent service delivery.
(1.2) The University College Hospital, Ibadan hereby Invites Interested competent and reputable Contractors/Suppliers/Consultancy Firms, with relevant experience and good track records of related projects and services to make submission for pre-qualification for the under listed projects:

(3) ELIGIBILITY REQUIREMENTS
Interested contractors /suppliers and Consultancy Firms are requested to submit the following documents/Information with verifiable evidence for assessment/consideration:
(a) Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including Form CAC1.1 or CAC2 and CAC7. Business Name with Form BN1 is also acceptable for Consultancy Services;
(b) Evidence of Company’s Income Tax Clearance Certificate (or Personal Income Tax Clearance Certificate of all the Partners in case of Business Name) for the last three (3) years valid till 31st December, 2023 i.e. 2020, 2021 & 2022;
(c) Evidence of Pension Clearance Certificate valid till 31st December, 2023 (this requirement is only applicable to bidders whose number of staff is 15 and above);
(d) Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2023 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is N50m and above);
(e) Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31stDecember, 2023;
(f) Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31st December, 2023 or valid Certificate issued by BPP.
(g) Sworn Affidavit:- disclosing whether or not any officer of the relevant committees of the Procurement Department, UCH Ibadan or the Bureau of Public Procurement is a farmer or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
– that no Director has been convicted in any Country for any criminal offence relating to fraud or financial impropriety or criminal misrepresentation or falsification of facts relating to any matter;
– that the Company is not in receivership, the subject of any farm of insolvency or bankruptcy proceedings or the subject of any form of winding up petition or proceedings;
– That the company does not have existing loan or financial liability with a bank, other financial institution or third party that is classified as doubtful, non-performance, bad or whose repayment portion has been outstanding for the last four (4) months;
(h) Company’s Audited Accounts for the last three (3) years – 2020, 2021 & 2022 duly acknowledge by the Federal Inland Revenue Services (FIRS);
(i) Reference Letter from a reputable Commercial Bank in Nigeria, indicating willingness to provide credit facility for the executor of the project when needed;
(j) Company’s Profile with the Curriculum Vitae of Kay Staff to be deployed for the project, including copies of their Academic/Professional qualifications such as COREN, ARCON, CORBON, QSRBN, COREN Mechanical, COREN Electrical SURCON etc.;
(k) Verifiable documentary evidence of at least three (3) similar jobs executed In the last five (5) years Including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects revealing the Commissioning of the projects;
(l) Works, List of Rants/Equipment with proof of Ownership/Lease Agreement;
(m) Supply and Installation of Equipment: Letter of Authorisation as Representatives of the Original Equipment Manufacturers (OEMs);
(n) For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided indicating among others the Lead Partner (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSTTF Clearance Certificate, IRR & Swam Affidavit are compulsory for each JV partner);
(o) All documents for submission must be transmitted with a Covering/ Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duty signed by the authorised officer of the firm;
Other mandatory requirements for Lots 1, 2, 3, 4, 5, 6, 7, 8, 9, 10 & 11 are listed as follows:
In respect of LOT 1-
(a) Bidders shall have minimum average annual turnover of not less than N300,000,000.00 (Three Hundred Million Naira Only) as evidence in the Tax Clearance Certificate,
(b) (I) Evidence of having at least one (1) COREN Registered Civil/ Structural Engineer with Practicing license valid for 2024 (copy of COREN Certificate and Current practicing License of the personnel must be attached).
(II) Evidence of having at least one (1) COREN Registered Mechanical Engineer with Practicing license valid for 2024 (copy of COREN Certificate and Current practicing License of the personnel must be attached).
(III) Evidence of having at least one (1) COREN Registered Electrical Engineer with Practicing license valid for 2024 (copy of COREN Certificate and Current practicing License of the personnel must be attached).
(IV) Evidence of having at least ore (1) QRSBN Registered Quantity Surveyor with practicing License valid for2024 (copy of QSRBN Certificate and Current practicing license of the personnel must be attached
In respect of LOT 2-
(a) Bidders shall have minimum average annual turnover of not less than N100,000,000.00 (One Hundred Million Naira Only) as evidence in the “Tax Clearance Certificate.
(I) Evidence of having at least one (1) COREN Registered Civil/ Structural Engineer with Practicing license valid for 2024 (copy of COREN Certificate and Current practicing License of the personnel must be attached).
(II) Evidence of having at least one (1) COREN Registered Mechanical Engineer with Practicing license valid for 2024 (copy of COREN Certificate and Current practicing License of the personnel must be attached).
(III) Evidence of having at least one (1) COREN Registered Electrical Engineer with Practicing license valid for 2024 (copy of COREN Certificate and Current practicing License of the personnel must be attached).
(IV) Evidence of having at least one (1) QRSBN Registered Quantity Surveyor with practicing License valid for 2024 (copy of QSRBN Certificate and Current practicing license of the personnel must be attached).
(v) Evidence of having at least one (1) ARCON Registered Architect with Practicing license valid for2024 (copy of ARCON Certificate and Current practicing License of the personnel must be attached
In respect of LOT 3-
(a) Bidders shall have minimum average annual turnover of not less than N50,000,000.00 (Fifty Million Naira Only) as evident In the “Tax Clearance Certificate.
(b) (I) Evidence of having at least one (1) COREN Registered Civil/Structural Engineer with Practicing license valid for 2024 (copy of COREN Certificate and Currant practicing license of the personnel must be attached).
(II) Evidence of having at least one (1) COREN Registered Mechanical Engineer with practicing license valid for 2024 (copy of COREN Certificate and Current practicing license of the personnel must be attached).
(III) Evidence of having at least one (1) COREN Registered Electrical Engineer with practicing license valid for 2024 (copy of COREN Certificate and Currant practicing license of the personnel must be attached).
(IV) Evidence of having at least one (1) QRSBN Registered Quantity Surveyor with practicing license valid for 2024 (copy of QSRBN Certificate and Current practicing license of the personnel must be attached
In respect of LOTS 4, 5, 6 & 7-
(a) Bidders shall have minimum average annual turnover of not less than N200,000,000.00 (Two Hundred Million Naira Only) as evident in the Tax Clearance Certificate.
(b)(I) Evidence of having at least one (1) COREN Registered Civil Engineer with practicing license valid for 2024 (copy of COREN Certificate and Current practicing license of the personnel must be attached).
(II) Evidence of having at least one (1) COREN Registered Mechanical Engineer with practicing license valid for 2024 (copy of COREN Certificate and Current practicing license of the personnel must be attached).
(III) Evidence of having at least one (1) QRSBN Registered Quantity Surveyor with practicing license valid for 2024 (copy of QSRBN Certificate and Current practicing license of the personnel must be attached.
(IV) Evidence of having at least one (1) CORBON Registered Builder with practicing license valid for 2024 (copy of CORBON Certificate and Current practicing license of the personnel must be attached.
(V) Evidence of having at least one (1) ARCON Registered Architect with Practicing license valid for 2024 (copy of ARCON Certificate and Current practicing License of the personnel must be attached
In respect of LOTS 4b, 5b & 6b –
(a) Bidders shall have minimum average annual turnover of not less than N200,000,000.00 (Two hundred Million Naira Only) as evident in the Tax Clearance Certificate.
(b) Letter of authorization from the original equipment manufacturer(OEM) listed in the approved policy far procurement of health and medical equipment for tertiary hospitals in Nigeria.
(c) Bidders must have verifiable documentary evidence as sole distributor / agent of the Original Equipment Manufacturers
In respect of LOT 8-
(a) Bidders shall have minimum average annual turnover of not less than N200,000,000.00 (Two Hundred Million Naira Only) as evident in the “Tax Clearance Certificate.
(b)(I) Evidence of having at least one (1) COREN Registered Civil Engineer with practicing License valid for 2024 (copy of COREN Certificate and Current practicing license of the personnel must be attached).
(II) Evidence of having at least one (1) COREN Registered Mechanical Engineer with Practicing license valid for 2024 (copy of COREN Certificate and Current practicing License of the personnel must be attached).
(III) Evidence of having at least one (1) QRSBN Registered Quantity Surveyor with practicing License valid for 2024 (copy of QSRBN Certificate and Current practicing license of the personnel must be attached.
(IV) Evidence of having at least one (1) CORBON Registered Builder with practicing license valid for 2024 (copy of CORBON Certificate and Current practicing license of the Personnel must be attached
In respect of LOTS 9 & 10-
(a) Bidders shall have minimum average annual turnover of not less than N60,000,000.00 (Sixty Million Naira Only) as evident in the “Tax Clearance Certificate.
(b) Evidence of Firm’s registration with National Information Technology Development Agency (NITDA) Computer Professionals Registration Council of Nigeria (CPN).
(c) Bidder must have verifiable documentary evidence as sole distributor/ agent of the Original Equipment Manufacturers.
In respect of LOT 11 –
(a) Bidders shall have minimum average annual turnover of not less than N100,000,000.00 (One Hundred Million Naira Only) as evident in the “Tax Clearance Certificate.
(b)(I) Evidence of having at least one (1) COREN Registered Civil/Structural Engineer with Practicing license valid for 2024 (copy of COREN Certificate and Current practicing License of the personnel must be attached).
(II) Evidence of having at least one (1) COREN Registered Mechanical Engineer with Practicing license valid for 2024 (copy of COREN Certificate and Current practicing License of the personnel must be attached).
(III) Evidence of having at least one (1) COREN Registered Electrical Engineer with Practicing license valid for 2024 (copy of COREN Certificate and Current practicing License of the personnel must be attached).
(IV) Evidence of having at least one (1) QRSBN Registered Quantity Surveyor with practicing License valid for 2024 (copy of QSRBN Certificate and Current practicing license of the personnel must be attached.
(V) Evidence of having at least one (1) ARCON Registered Architect with Practicing license valid for 2024 (copy of ARCON Certificate and Current practicing License of the personnel must be attached
Note: All pages of document submitted must: be serially arranged and authenticated by the company’s CEO or his/her representative, failure will lead to disqualification.
SUBMISSION OF EOI AND PRE-QUALIFICATION DOCUMENTS
Interested firms are to submit three (3) bound copies for Expression of Interest (EOI) for “the engagement of a Consultant” which is applicable to Lots 2,4,5, 6 & 7 and Pre-qualification documents in two (2) Hard copies and one (1) soft copy as applicable, separated by dividers and arranged as indicated above. The documents should be submitted in a sealed envelope and addressed to The Chief Medical Director, University College Hospital, Ibadan and clearly marked with (the name of the project and Lot number) e.g. Lot 1 General Renovation of the Hospital.
Furthermore, the reverse of each sealed envelope should bear the name and address of the bidder and drop it in the designated Tender Box at the office of The Assistant Director, Procurement University College Hospital, Ibadan not later than 12:00 noon Monday, 18 March, 2024.
(5) OPENING OF EOI AND PRE-QUALIFICATION DOCUMENTS
The EOI and Pre-qualification documents will be opened immediately after the deadline for submission at 12:00 noon Monday, 18 March, 2024, at The Procurement Department Workstation, UCH, Ibadan.
(7) IMPORTANT NOTICE
Pre-qualified Consultants will be requested to pay N10,000.00 (Ten Thousand Naira Only) processing fee before collecting the Request for Proposal (RfP).
(8) GENERAL INFORMATION!.
(I) The Bidders are expected to adhere strictly to ALL instruction as non-compliance will constitute a ground for disqualification.
(II) EOIs/Bids must be in English language and signed by an official authorised by the bidders.
(III) The EOIs/bids will be opened in the presence of Management and Professional bodies and covered by video recording;
(IV) Invitation for Pre-qualification: Only pre-qualified bidders at technical evaluation will be invited at a later date for collection of tender documents;
(V) Expression of Interest (Services): Only shortlisted Consultants will be Invited at a later date for collection of Request for Proposals (RfP);
(VI) Tendering with false document Is an offence, which will lead to disqualification and prosecution.
(VII) All cost will be borne by the bidder.
(VIII) The University College Hospital, Ibadan, is not bound to pre-qualify/shortlist any bidder and reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act, 2007.
