INVITATION FOR BIDS (IFB)
LSMOH- RENOVATION OF THE TEN (10) STOREY DOCTORS’ QUARTERS AT GENERAL HOSPITAL, ODAN
IFB: LSMOH/W/NCB/001/23
(1.0) BACKGROUND
The Lagos State Ministry of Health has secured necessary approval to execute renovation project in the fulfilment of the THEME+ Agenda. The State in its commitment to improving healthcare delivery, has therefore embarked on the renovation of the ten storey Doctors Quarters, at General Hospital Odan.
(2.0) This National Competitive Bidding (NCB) process as specified by the State Procurement Guidelines is open to all bidders from eligible countries as defined in the Guidelines –www.lagosppa.gov.ng
(3.0) SCOPE OF WORK
The Lagos State Ministry of Health now invites sealed bids from eligible contractors for the contract below as detailed in the bidding document:

(4.0) GENERAL QUALIFICATION REQUIREMENTS FROM INTERESTED PARTIES
(i) Evidence of Registration with Corporate Affairs Commission (CAC) with inclusion of Certificate of Incorporation and Articles of Association.
(ii) Company profile and Organisational structure with full name of organisation and contact person, postal address, telephone/fax numbers, and e-mail addresses.
(iii) Evidence of company tax clearance for the last 3 years (2020, 2021 & 2022)
(iv) Audited financial statements of the company for the years 2019, 2020 & 2021.
(v) Evidence of 3 years development levy for two directors.
(vi) Copies of VAT Registration with TIN number and evidence of VAT remittances for the last 3years.
(vii) Evidence of available financing and/or access to credit line.
(viii) Verifiable evidence of 2 similar job experiences (within a minimum of 5 years) with government, private or non-governmental organizations. (These should include evidence of satisfactory completion or progress for each experience indicated).
(ix) A Sworn Affidavit that all documents submitted is genuine and verifiable.
(x) Evidence of registration with Lagos State Government (PPA) in the Appropriate Class
(5.0) NOTES:
(a) Additional details are provided in the Bidding Documents.
(b) All claims must be adequately substantiated with documents and must be verifiable.
(c) The Ministry of Health shall deal directly with only the authorized officers of interested companies and not their agents.
(d) This bidding exercise shall not be construed as a commitment on the part of the Ministry of Health to award the contracts to anyone or even award any contracts at all.
(e) The Ministry of Health will not be responsible for any cost or expenses incurred by any interested party(ies) in connection with site inspections or responses to this invitation.
(6.0) SPECIFIC TECHNICAL EVALUATION CRITERIA
(6.1) RENOVATION OF THE TEN (10) STOREY DOCTORS’QUARTERS AT GENERAL HOSPITAL, ODAN
(i) Experience as a prime Contractor in carrying out 2-3 project(s) of similar nature and complexity equivalent to the works over the last five (5) years (to comply with this requirement, the work cited should be at least 70% completed) including the following details:

· Clients: Nature of Contract:
· Name and address of a referee in client’s company:
· Letter of contract award
· Certificate of Contract completion
(ii) Documentary evidence of technical equipment and key personnel to execute the job.
(iii) The Bidder shall provide details of the proposed personnel and their experience records that meets standard requirement.
(7.0) COLLECTION OF BID DOCUMENTS
(7.1) Only Electronic e-Copy of Bid Document will be available on the Lagos State Public Procurement e-Bidding Platform (R2Cow Biddingo)
(8.0) SUBMISSION OF BID DOCUMENTS
(8.1) The Bidder shall:
BID SUBMISSION SHALL BE BY e-COPY UPLOADED TO THE WEBSITE (the website is the (centralized) Lagos State Public Procurement Agency e-Portal (R2cow Biddingo).
(8.2) All bids should be accompanied with a bid security of 1% of bid price from a reputable Commercial Bank or Lagos State Government Recognized Insurance Company
(9.0) BID OPENING
(9.1) Bids will be opened online via zoom in the presence of the bidder or bidder’s representative who chose to log on at 12:00pm on Tuesday 7 November, 2023
(9.2) ADDITIONAL INFORMATION
All request for clarifications should be directed to the Head of Procurement via email at orolatunji@lagosstate.gov.ng on or before 12:00pm Wednesday 1st November, 2023.
(9.3) DELIVERY ADDRESS:
BID SUBMISSION SHALL BE BY e-COPY UPLOADED TO THE WEBSITE (the website is the (centralized) Lagos State Public Procurement Agency e-Portal (R2cow Biddingo).
(9.4) Completed bids should be uploaded to the website on or before 12:00pm Friday 3rd November, 2023.
NB: Successful bidder shall be required to renew its registration for year 2020 with the Lagos State Public Procurement Agency (PPA) on notification.
