FEDERAL TEACHING HOSPITAL KATSINA

(1)          INTRODUCTION

(1.1)       In compliance with the requirements of the Public Procurement Act, 2007, the Federal Teaching Hospital, Katsina hereby invites reputable Companies/Contractors with proven experience to undertake the under listed 2024 Capital Projects.

(2)          SCOPE OF WORKS/SUPPLY

(2.1)       GOODS:

LOT 1 – Procurement and Installation of modular theater equipment.

LOT 2 – Procurement and Installation of anesthesia equipment

LOT 3 – Procurement and Installation of anatomic and molecular pathology equipment

LOT 4 – Procurement and Installation of mortuary equipment.

LOT 5 – Procurement of various ICT equipment

LOT 6 – Procurement of IPC Materials.

LOT 7 – Procurement and Installation of public health laboratory equipment

LOT 8 – Procurement of a Hysteroscopy/TCRIS tower.

LOT 9 – Procurement and Installation of neuro-surgical equipment

LOT 10 – Procurement of various operation instruments.

LOT 11 – Procurement of psychiatry equipment

LOT 12 – Procurement of ENT equipment.

LOT 13 – Procurement of Paediatrics equipment

LOT 14 – Procurement and Installation of Ophthalmology equipment

LOT 15 – Procurement and Installation of Maternity equipment

LOT 16 – Procurement of Molecular Laboratory equipment

(2.2)          WORKS:

LOT 17 – Construction of generator house with procurement and installation of a 500KVA generator.

LOT 18 – Trenching/Installation of ICT fiber-optic cables and CCTV cameras.

LOT 19 – Renovation of Guest houses.

LOT 20 – Renovation of laundry and construction of a De-Montfort incinerator.

LOT 21 – Renovation of NHIS Clinic.

LOT 22 – Renovation of IPC complex

LOT 23 – Renovation of House Officer’s Quarters.

LOT 24 – Renovation of Mortuary.

LOT 25- Construction of Transformer Plinth/Housing and Supply/installation of 1MVA/11/415KV Distribution Transformer with all accessories.

LOT 26 – Installation of solar panels and inverters.

LOT 27 – Renovation of clinics and offices.

(3)          ELIGIBILITY REQUIREMENTS

(a)          Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including Form CAC1.1 or CAC2 and CAC7

(b)          Evidence of Company’s Income Tax Clearance Certificate for the last three (3) years valid till 31st December, 2024;

(c)           Evidence of Pension Clearance Certificate valid till 31st December, 2024 (this requirement is only applicable to bidders whose number of staff is 15 and above);

(d)          Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2024 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is N50m and above);

(e)          Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2024;

(f)           Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31st December, 2024 or valid Certificate issued by BPP.

(g)          Sworn Affidavit:

·         disclosing whether or not any officer of the relevant committees of the Federal Teaching Hospital, Katsina or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and corned in all particulars;

·         that no Director has been convicted in any Country for any criminal offence relating to fraud or financial impropriety or criminal misrepresentation or falsification of facts relating to any matter

·         that the Company is not in receivership, the subject of any form of insolvency or bankruptcy proceedings or the subject of any form of winding up petition or proceedings;

·         That the company does not have existing loan or financial liability with a bank, other financial institution or third party that is classified as doubtful, non-performance, bad or whose repayment portion has been outstanding for the last four (4) months;

(h)          Company’s Audited Accounts for the last three (3) years – 2021, 2022 & 2023;

(i)           Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;

(j)           Company’s Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications such as COREN, QSRBN, ARCON, CORBON, MDCN, etc.;

(k)          Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects;

(l)           Works: List of Plants/Equipment with proof of Ownership/Lease Agreement;

(m)         For procurement and supply of medical equipment, the medical equipment must conform to the approved policy for procurement of health and medical equipment/list of manufacturers and equipment for tertiary hospitals published by the BPP/FMOH/SON/NNRA, year 2022-2024 (3rd Revision);

(n)          For ICT jobs: Evidence of current Licence issued by National Information Technology Development Agency (NITDA);

(o)          For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided indicating among others the Lead Partner (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR& Sworn Affidavit are compulsory for each JV partner);

(p)          All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorised officer of the firm.

(4)          COLLECTION OF TENDER DOCUMENTS

Interested companies are to collect the Standard Bidding Documents (SBD) from the office of the Head of Procurement, Federal Teaching Hospital, Katsina, between the hours of 5am to 4pm daily (Monday to Friday) upon payment of non-refundable tender fee of Ten Thousand Naira Only (N10,000.00) paid into the Federal Teaching Hospital Katsina  TSA in any reputable Commercial Bank in Nigeria.

(5)          SUBMISSION OF TENDER DOCUMENTS

Prospective bidders are to submit two (2) hard copies each of the technical and financial bids with softcopy of the financial bid in MS Excel format, packaged separately in sealed envelopes and clearly marked as “Technical Bid” and ‘Financial Bid”. Thereafter, the two sealed envelopes should be put together in a bigger sealed envelope addressed to the Chief Medical Director, Federal Teaching Hospital Katsina, Murtala Muhammad Way, P.M.B 2121, Katsina. The sealed envelope should be marked with the name of the project and Lot number. The reverse side of each sealed envelope should have the name and address of the bidder. The sealed envelope should be dropped in the designated tender box in the office of the Chief Medical Director not later than 12:00 noon, Monday, March 4, 2024.

(6)          OPENING OF TECHNICAL BIDS

Only the Technical bids will be opened immediately after the deadline for submission at 12:00 noon, Monday, March 4, 2024, in the conference hall of the Federal Teaching Hospital Katsina, Katsina in the presence of bidders or their representatives. Prospective bidders should ensure they sign the Bid Submission Register in the office of the Chief Medical Director, Federal Teaching Hospital Katsina, Katsina. (7)          GENERAL INFORMATION

(a)          Bids must be in English Language and signed by an official authorised by the bidder;

(b)          Bids submitted after the deadline for submission would be returned un-opened;

(c)           Bidders should not bid for more than two (2) Lots;

(d)          All costs will be borne by the bidders;

(e)          Only pre-qualified bidders at technical evaluation will be invited at a later date for financial bids opening, while the financial bids of un-successful bidders will be returned un-opened;

(f)           The Federal Teaching Hospital Katsina reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement Act, 2007.
