INVITATION FOR TENDER/PREQUALIFICATION/EXPRESSION OF INTEREST (EOI) FOR 2024 CAPITAL PROJECTS

1. INTRODUCTION
The Federal Teaching Hospital, Ido-Ekiti, a tertiary health institution, in implementation of her 2024 Appropriation Act for capital projects and in compliance with the Public Procurement Act, 2007 hereby invites all interested, qualified and reputable contractors and consultants with relevant experience and track record of excellence previous performance to tender for the following projects.

2. SCOPE OF WORKS/GOODS/SERVICE

CATEGORY A: WORKS
LOT NO DESCRIPTION
Lot 1 Construction of conference centre at Federal Teaching Hospital, Ido-Ekiti
Lot 2 Construction of perimeter fence at Federal Teaching Hospital, Ido-Ekiti
CATEGORY B: GOODS
Lot 3 Procurement of furniture and fittings of office use
Lot 4 Procurement and installation of surgical equipment
Lot 5 Procurement and installation of laboratory equipment
Lot 6 Equipping new accident and emergency theatre
Lot 7 Procurement and installation of internal medicine equipment
Lot 8 Provision of central sterilizing unit.
CATEGORY C: CONSULTANCY SERVICE
Lot 9 Capacity building for clinical and non-clinical staff

3. ELIGIBILITY REQUIREMENTS
For the purpose of evaluation, interested contractors/consultants should submit the following documents
a) Evidence of certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Forms CAC1.1 or CAC2 and CAC7. Business name with form BN1 is also acceptable for services;
b) Evidence of firm’s Tax Clearance Certificates of all the partner in case of business name for the last three years valid till 31st December, 2023,
c) Evidence of Pension Clearance Certificate valid till 31st December, 2023 (this requirement is only applicable to bidders whose number of staff is 15 and above).
d) Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2023 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is N50m and above);
e) Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2023;
f) Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2023 or valid Certificate issued by BPP;
g) Sworn affidavit disclosing

  • disclosing whether or not any officer of the relevant committees of the Federal Teaching Hospital, Ido-Ekiti or Bureau of Public Procurement (BPP) is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
  • that no director has been convicted in any country for any criminal offence relating to fraud or financial impropriety or criminal misrepresentation or falsification of facts relating to any matter;
  • that the company is not receivership, the subject of any form of insolvency or bankruptcy proceedings or he subject of any form of winding up petition or proceedings;
  • That the company does not have existing loan or financial liability with a bank, other financial institutions of third party that is classified as doubtful, non-performance, bad or whose repayment portion has been outstanding for the last four (4) months.

h) Company’s Audited Accounts for the last three years (2020, 2021 & 2022);
i) Reference letter from a reputable commercial bank indicating willingness to provide credit facility for the execution of the project when needed;
j) Company’s profile with the curriculum vitae of key staff to be deployed for the project, including copies of their Academic/Professional certificates such as COREN, QSRBN, ARCON, CORBON etc
k) Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including letters of awards, valuation certificates, job completion certificates and photographs of the projects;
l) Works: List of Plants/Equipment with proof of Ownership/Lease agreement;
m) Lots 4 to 8: letter of authorization from the original equipment manufacturers listed in the approved policy for procurement of health and medical equipment for tertiary hospitals in Nigeria;
n) Lot 3: Letter of authorization from the Original Equipment Manufacturers (OEM);
o) Lot 9: evidence of firm’s current registration with the relevant regulatory professional bodies such as MDCN, CMD etc;
p) For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificates, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn affidavit are compulsory for each JV Partner);

Note: All documents for submission must be submitted with a Covering/Forwarding letter under the Company’s letter head paper bearing amongst others, the Registration Number (RC) as issued by the CAC, Contact Address, Telephone Number (preferably GSM No) fax and email address. The letter head paper must bear the Names and Nationalities of the Directors of the company at the bottom of the page, duly signed by the authorized officer of the firm. The EOI/Technical bids should be orderly and well paginated with table of contents and follow the order listed above, failure to comply with this directive will result in disqualification of the bidder.

4. COLLECTION OF TENDER DOCUMENTS (GOODS & WORKS)
The tender documents are to be collected from procurement office II Procurement Department Building FTH, Ido Ekiti between the working hours of 8:30am to 4:00om daily upon the payment of non-refundable tender fee of N10, 000.00 per LOT into the Federal Teaching Hospital, Ido-Ekiti’s REMITA’s account and presentation of the copy of the Hospital’s receipt. The photocopy of the receipt should be attached to the tender documents.

5. SUBMISSION OF TENDER DOCUMENTS
Completed tender document should be two hard copies (Marked “Original & Copy) each for both Technical & Financial Bids with soft copy of financial bid only in MS Excel format, packaged in two separate sealed envelopes clearly marked at the top right corner of each envelope stating the title of the project i.e. TECHNICAL BIDS FOR LOT 2 & FINANCIAL BIDS FOR LOT 2, and at the reverse side the name of the company should boldly be written. Both envelopes shall then be put in a large envelope clearly marked at the top right corner stating the title of the project and the name and address of the company at the reverse side and be left in the appropriate tender box at procurement office (Room 13, Administrative building) to be addressed to:

6. SUBMISSION OF PREQUALIFICATION DOCUMENTS (ONLY FOR LOT 1 ONLY IN CATEGORY A: WORKS)
Prospective bidders are to submit pre-qualification documents for each of the lot desired, two hard copies each of technical packaged in sealed envelopes and clearly marked at the top right corner of each envelope stating the title of the project i.e ‘TECHNICAL BIDS FOR LOT 1” and at the reserve side the name and address of the company should boldly be written and be left in the appropriate tender box at procurement office, II, procurement department building to be addressed to:

7. SUBMISSION OF EOI (CATEGORY C)
Interested firms are to submit Expression of Interest (EOI) documents for the lot desired; two hard copies each of EOI packaged in sealed envelopes and clearly marked at the top right corner of each envelope stating the title of the project i.e “EXPRESSION OF INTEREST FOR LOT 9”, and at the reverse side the name and address of the company should boldly be written and be left in the appropriate tender box at procurement office II, Procurement department building to be addressed to:

The submission should be made not later than 12:00 noon on Monday, 4th March, 2024. Late application will be returned unopened. 8. OPENING OF EOIS AND PRE-QUALIFICATION DOCUMENTS
The EOI/technical bids will be opened immediately after the deadline for submission at 12:00 noon on Monday, 4th March, 2024 at the Hospital’s MINI CONFERENCE HALL.

9. OPENING OF TECHNICAL BIDS
Only the technical bids will be opened immediately after the deadline for submission at 12:00 noon on Wednesday 27th March, 2024 at the Hospital’s MINI CONFERENCE HALL while the financial bids will be kept un-opened.

10. NOTATION
i. Bids must be in English Language and signed by an official authorized by the bidder;
ii. All documents (Sensitive and non-sensitive) submitted would be verified. Bidders are advised to ensure integrity of all documents submitted
iii. The evaluation of the received documents shall be strictly based on the requirements specified in this Invitation to Tender.
iv. Any financial bids without softcopy(ies) Specified format shall not evaluated
v. Bidders should not bid for more than two (2) lots
vi. This publication is for information purpose only and does not constitute an offer by the Hospital to transact with any party for the project, nor does it constitute a commitment or an obligation on the part of the Hospital to award the contract to any contractor or agent.
vii. The Hospital will not be responsible for any cost of expenses incurred by any interested company(ies) in connection with any response to this invitation and/or preparation or submission of the documents.
viii. The bids opening will be covered by video recording (link will be sent to bidders who may not be able to attend) with the presence of two (2) representation of private sector professional bodies and civil society organization in the areas of Anti-corruption/Transparency;
ix. Bid document can also be collected in soft copies and Bidders can submit through Post (Courier Services)
x. Lot 2-8: Only pre-qualified bidders at technical evaluation will be invited at later date for virtual financial bids opening, while the financial bids of un-successful bidders will be returned un-opened;
xi. Lots 1 & 9: only pre-qualified/shortlisted bidders at technical/EOI evaluation will be invited at a later date for collection of tenders documents/request for proposal;
xii. The FTH, Ido-Ekiti is not bound to prequalify and bidder reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement ACT 2007.
