FEDERAL MEDICAL CENTRE, EBUTE-METTA, LAGOS

REQUEST FOR EXPRESSIONS OF INTEREST (EOI) FOR THE ENGAGEMENT OF CONSULTANCY FIRM FOR MODERN CANCER TREATMENT EQUIPMENT FOR ONCOLOGY CENTRE, FEDERAL MEDICAL CENTRE, EBUTE-METTA, LAGOS

(1.0)  INTRODUCTION

The Federal Medical Centre, Ebute-Metta, Lagos in implementing her year 2023 Capital Projects plans to embark on equipping her Oncology Centre, to make specialized healthcare treatment, diagnosis, investigations, testing and treatment of cancerous diseases readily available to Nigerians hereby invites reputable Consulting Firms with relevant experience for the specifications, supervision of the under listed Projects list in 2.0.

CONSULTANCY FOR SERVICES

SCOPE includes supervision over:

·        Linear Accelerator + Accessories (External Beam Radiotherapy Equipment)

·        Construction of Bunkers/Building (Radiotherapy Building)

·        Brachytherapy Equipment and Large Bore CT Simulation Equipment

(3.0)  ELIGIBILITY REQUIREMENTS

Interested Firms are required to submit the following documents in sequential order as listed below

(a)      Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including Form CAC1.1, or CAC2, and CAC7. Business Name with Form BN1 is also acceptable;

(b)     Evidence of Company’s Income Tax Clearance Certificate (or Personal Income Tax Clearance Certificate of all the Partners in case of Business Name) for the last three (3) years valid till 31st December, 2023;

(c)     Evidence of Pension Clearance Certificate valid till 31st December, 2023 (this requirement is only applicable to bidders whose number of staff is 15 and above);

(d)     Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2023 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is N50m and above);

(e)      Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2023;

(f)      Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31st December, 2023 or valid Certificate issued by BPP.

(g)     Sworn Affidavit;

–         disclosing whether or not any officer of the relevant committees of the Federal Medical Centre, Ebute-Metta, Lagos or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;

–         that no Director has been convicted in any Country for any criminal offence relating to fraud or financial impropriety or criminal misrepresentation or falsification of facts relating to any matter;

–         that the Company is not in receivership., the subject of any form of insolvency or bankruptcy proceedings or the subject of any form of winding up petition or proceedings;

–         that the company does not have existing loan or financial liability with a bank, other financial Institution or third party that is classified as doubtful, non-performance, bad or whose repayment portion has been outstanding for the East four (4) months;

(h)     Company’s Audited Accounts for the last three (3) years – 2020, 2021 & 2022;

(i)      Evidence of valid membership of the relevant professional body for Clinical and Radiation Oncology in Nigeria by the Principal Consultant;

(j)      Reference Letter from a reputable Commercial Bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;

(k)     Company’s Profile with the Curriculum Vitae of Firm’s Consultants to be deployed for the project, including copies of their Academic/Professional qualifications such as Oncology, Radiotherapy, etc.;

(l)      Evidence that the firm has expertise in Oncology, Radiotherapy and Medical Physics in the last twenty (20) years’ in Nigeria;

(m)    Evidence that the firm has offered at least three (3) such consultancy in the last 5 years in Nigeria;

(n)     For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner);

(o)     Note: All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No), and e-mail address, The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorised officer of the firm.

(4.0)  SUBMISSION OF EXPRESSION OF INTEREST DOCUMENT

(i)      Prospective Firms are to submit two (2) hard copies each of the EOI which are to be packaged in sealed envelopes and clearly marked ‘Original” & “Copy’ and thereafter put together in a bigger sealed envelope indicating lot/project number, title, name and address of the Bidder at the top left corner of the envelope with bidders GSM (reachable) Number(s) and email Addresses(s);

(ii)      Please, ensure that you sign the EOI Submission Register as the Federal Medical Centre, Ebute-Metta will not be held liable for misplaced or wrongly submitted bids;

(iii)     All documents should be submitted not later than 12:00 noon Monday, 18 December, 2023;

(iv)     The envelopes should be addressed to THE MEDICAL DIRECTOR, FEDERAL MEDICAL CENTRE, EBUTE-METTA, LAGOS and dropped in the Tender Box located in the office of the HEAD OF PROCUREMENT.

(5.0)  OPENING OF EOI

(i)      The Expressions of Interest will be opened immediately after the deadline for submission at 12:00 noon Monday, 18 December, 2023, in the Seminar Room, Federal Medical Centre, Ebuta- Metta, Lagos;

(ii)      All Tenderers, Civil Society Organisation and interested stakeholders are invited to the bid opening;

(6.0)  GENERAL INFORMATION

(i)      EOIs must be in English language and signed by an official authorised by the bidder

(ii)      Late submission will be returned un-opened;

(iii)     All costs incurred in responding to this advertisement shall be borne solely by bidders;

(iv)     Only shortlisted Firms will be invited at a later date for collection of Request for Proposal;

(v)     If a firm is in joint venture (JV) partnership or Consortium agreement, the role of each partner must be clearly defined in the JV agreement. Additionally, the Memorandum of Understanding, (MOU) must specify Lead Partner.

(vi)     This EOI shall neither be construed as a commitment on the part of Federal Medical Centre, Ebute- Metta, Lagos nor shall it entitle any contractor to make any claim whatsoever and/or seek any indemnity from FMC, Ebute-Metta, Lagos;

(vii)    The EOIs will be opened in the presence of Bidders or their representatives, CSOs in the areas of Anti-Corruption and Transparency and Private Professional bodies; and will be covered by video recording: invitation link will be sent to bidder who chooses to join the EOI’s opening online;

(viii)   The Federal Medical Centre, Ebute-Metta, Lagos is not bound to shortlist any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement Act, 2007.
